This third party service can be used to report complaints or concerns regarding harassment, discrimination, or retaliation under our Anti-Discrimination and Anti-Harassment and Complaint Procedure Policies, as well as complaints or concerns regarding any other misconduct, such as fraud, theft, misuse of funds, conflict of interest, and so forth. Any matters reported to this third party service will be handled according to the procedures set out in those policies.
In the box labeled “Subject,” please identify the subject matter (for example, “harassment complaint” or “bias concern” or “fraud report”), and then in the box labeled “Description” please provide a fuller description of the complaint or concern you are reporting. To enable the company to undertake a prompt investigation and take action to address the matter raised, please provide as much detail as you can, including a detailed description of the situation or conduct that is the focus of the complaint or concern, the names of all individuals involved, and any witnesses.
Lastly, at your sole option, you can leave your email address to allow the company to respond directly to your concerns. Please note your email address is NOT disclosed to the company; it simply allows the company to open a direct and fully anonymous line of communication with you to discuss your complaint or concern. If you are not interested in receiving a direct reply from the company or otherwise do not feel comfortable sharing your email anonymously, you are 100% free to omit your email address from this submission.
